Your Right to a Good Faith Estimate
Notice to Self-Pay Clients and Prospective Clients:
Under Section 2799B-6 of the Public Health Service Act (commonly known as the No Surprises Act), health care providers are required to inform individuals who are not enrolled in an insurance plan or coverage, or who are not using an insurance plan, an estimate of the bill for medical items and services.
- Right to Receive an Estimate: You have the right to receive a Good Faith Estimate explaining the total expected cost of any non-emergency items or services, including psychotherapy sessions.
- Timing of Delivery: Your health care provider must give you a Good Faith Estimate in writing at least one (1) business day before your medical service or item is provided. You can also request an estimate prior to scheduling care.
- Dispute Resolution: If you receive a bill from Mindful Tide Counseling PLLC that is at least $400 more than your Good Faith Estimate, you have the legal right to dispute the bill.
- Save Your Copy: Please save a copy or picture of your Good Faith Estimate for your records.
